Might of the Revenant - drops off side / front bosses
Mnemonic Fragment - drops off Dominus wtfever his name is, front, and the immortal
Permafrost - drops off the Inobak
Skull of Despair - side bosses
Soul Siphon - doubt it drops in IS
Teachings of the Immortal One - side bosses
Third Circle of Inner Sanctum - all over on lvl 3
Gloomfall armor parts - also all over
Gelid Blade of Inobak - Inobak

This is what I have verified from the Inner Sanctum bosses (I'm missing a few random items like rings and shoulder-wear which are common (and upgrades too), and of course there are items that drop that I haven't listed because I haven't seen them drop yet -- i.e. the primeval skull everyone says drops

bloodmark & soulmarks drop like this:

1st floor: bloodmark & soulmark
2nd floor: tainted bloodmark & tainted soulmark
3rd floor: desecrated bloodmark & desecrated soulmark

Preceptors are the ones dropping these.

I have only heard of one permafrost ever dropping, not seen it with my own eyes. Supposedly Inobak drops it.

Inobak drops the gold card: essence.

Skull of misery & despair can be confirmed, I have seen both drop and have a skull of despair myself.

We will probably kill Hezak again later today, perhaps his loot has been fixed in the patch yesterday. Will post an update on what he drops if we kill him.

This really makes me wonder what Hezak drops that doesn't drop from the other mobs in the same room. The only thing I can see so far is the Ichor of the Immortal One out of our two Hezak kills. That's very pathetic.
